Forms are completed online through the patient portal. After completing your deposit for the initial session, a personal link will be emailed to you to create an online account and complete all new patient forms. Please make sure these forms are completed prior to the first appointment. Forms will be reviewed at the first session.
For your convenience, our initial forms are also available for you to download and print out. If forms are not completed in the patient portal ahead of your appointment, then please print, complete the forms prior to arriving for your first session, and bring the forms to your first session. This ensures your session can begin and end on time. We will discuss all of them in detail and answer any questions at our first meeting. Thank you.
